Join Our Team as a Case Manager RN PRN

We are seeking a skilled and compassionate Case Manager RN PRN to join our team at HCA Florida Fawcett Hospital. As a Case Manager RN PRN, you will play a vital role in ensuring that our patients receive high-quality, patient-centered care.

About the Role

The Case Manager RN PRN is responsible for managing rehabilitative care to ensure optimum outcomes for our patients. This includes coordinating efforts within the rehabilitation team, collaborating with the rehabilitation program director and facility case management director, and ensuring that patient treatment plans are implemented effectively.

Responsibilities
  • Implement patient treatment plans that support patients' strengths, abilities, needs, and preferences
  • Facilitate patient involvement throughout the rehabilitation process
  • Document findings of discharge planning evaluations and psychosocial assessments
  • Communicate social, financial, or discharge needs and preferences of patients/families/caregivers
Requirements

To be successful in this role, you will need:

  • Completion of an RN diploma program or related healthcare program
  • Minimum of 3 years recent acute clinical practice or related healthcare experience
  • Critical thinking, service excellence, and good interpersonal communication skills
  • Ability to read/comprehend written instructions and follow verbal instructions
  • PC skills and analytical and critical thinking abilities with proactive decision-making and negotiation skills
