Job Summary

Key Responsibilities
  • Conduct thorough assessments of patients' needs and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless transitions of care
  • Provide education and support to patients and their families
  • Manage patient flow and coordinate care to optimize outcomes
  • Develop and maintain relationships with community resources and healthcare providers
Requirements
  • Graduate from an accredited school of nursing (RN) required, bachelors preferred
  • Two (2) or more year's clinical experience preferred
  • Acute care or community social agency case management experience preferred
  • Must possess good assessment, intervention, and problem-solving skills
  • Ability to communicate effectively and efficiently, verbally and in writing
Preferred Qualifications
  • Case Management Certification (CCM) preferred
  • Bilingual and/or English-Spanish speaking abilities preferred
