Job Summary

The Economic Opportunity Council of Suffolk Inc. is seeking a highly skilled Care Coordination Specialist to join our team.

The successful candidate will be responsible for coordinating comprehensive medical and behavioral health care for patients with chronic conditions, ensuring access to appropriate services and improving health outcomes.

This role requires strong organizational skills, cultural sensitivity, and the ability to work effectively with diverse groups and stakeholders.

The estimated salary for this position is $55,000 per year, based on industry standards and location.

Key Responsibilities
  • Develop and implement individualized service plans to achieve clients' short- and long-term goals.
  • Coordinate all services received by clients from multiple providers, including healthcare, social services, and community resources.
  • Provide patient and family support, education, and advocacy to promote optimal health outcomes.
Requirements
  • Bachelor's degree in Social Service or Counseling Discipline.
  • Two to five years of experience in direct services to mentally disabled, chronically ill, or other underserved populations.
  • Bi-lingual (Spanish) preferred.
  • A valid driver's license and insured vehicle with 100/300 bodily injury insurance.
  • PPD (TB test required with proof of results).
