About the Job:

Economic Opportunity Council of Suffolk Inc is seeking a highly motivated and dedicated Community Health Outreach Specialist to join our team. This is a dynamic role that requires strong communication and interpersonal skills, as well as the ability to work effectively in a multicultural environment.

Job Summary:

The Community Health Outreach Specialist will be responsible for providing culturally competent client support and advocacy, conducting community outreach to identify, engage and re-engage clients into care/services and to market program services. The ideal candidate will have experience in peer education outreach, group facilitation, volunteer coordination or support, and/or community organizing/mobilization.

Responsibilities:

  • Client Support and Advocacy:
    • Provide culturally competent client support and advocacy to help clients navigate the healthcare system and access necessary services.
    • Conduct home visits, hospital visits, or other site visits to provide support and advocacy services.
  • Community Outreach:
    • Develop and implement effective community outreach strategies to identify, engage and re-engage clients into care/services and to market program services.
    • Collaborate with local community organizations, healthcare providers, and other stakeholders to ensure seamless service delivery.
  • Peer Education and Training:
    • Develop and deliver peer education and training programs to promote health literacy and disease prevention among target populations.
    • Participate in community events, conferences, and workshops to promote the organization's mission and services.
  • Data Management:
    • Maintain accurate and up-to-date records of client interactions, service delivery, and outcomes.
    • Analyze data to inform program development, policy-making, and resource allocation decisions.
  • Professional Development:
    • Pursue ongoing professional development opportunities to enhance knowledge and skills in areas such as HIV/AIDS care, harm reduction, and cultural competency.
    • Participate in regular supervision and coaching sessions to ensure optimal performance and job satisfaction.

Requirements:

  • Bachelor's degree in a relevant field (e.g., social work, public health, psychology).
  • At least 2 years of experience in peer education outreach, group facilitation, volunteer coordination or support, and/or community organizing/mobilization.
  • Demonstrated expertise in HIV/AIDS care, harm reduction, and cultural competency.
  • Excellent verbal and written communication skills, including strong interpersonal and organizational skills.
  • Familiarity with Microsoft Office Suite and data management systems.
  • Valid New York driver's license and insured vehicle.

Compensation:

$55,000 - $65,000 per year, depending on experience.
