  • Provide clinical and case management support to individuals in a mobile crisis intervention team.
  • Demonstrated experience in addressing co-occurring substance use and mental health challenges is essential.
  • Ensure prompt and accurate documentation of services rendered.
  • Facilitate individual, group, and family therapeutic sessions.

Required Qualifications:

  • A Master's degree in Social Work is mandatory.
  • New York State Registration/Certification as a Social Worker is required.
  • Previous experience in a healthcare or community service setting is advantageous.
  • Familiarity with the target patient demographic and the use of PM Schedule along with LMSW or LCSW credentials is crucial.
  • Experience in managing co-occurring substance use and mental health disorders is a prerequisite.
