Job Title: Clinical Documentation Specialist

We are seeking a highly skilled Clinical Documentation Specialist to join our team at Loma Linda University Medical Center. As a Clinical Documentation Specialist, you will play a critical role in ensuring the accuracy and completeness of patient medical records.

Key Responsibilities:
  • Conduct concurrent, prospective, and retrospective clinical documentation reviews to ensure compliance with Risk Adjustment Data Validation (RADV) timelines.
  • Communicate with providers to clarify documentation needs and provide education on documentation integrity, completeness, and consistency.
  • Stay up-to-date on CMS, ICD-10-CM, AHA Coding, and health plan guidelines related to Risk Adjustment.
  • Perform other duties as needed to support the team.
Requirements:
  • Bachelor's degree in Health Information Management or a related field.
  • 2+ years of combined clinical/medical experience, with at least 1+ year of Risk Adjustment coding specific experience.
  • Certified Risk Adjustment Coder (CRC) certification required.
