Job Summary

The RN or LPN Case Manager is a key member of our healthcare team at The Highlands, responsible for coordinating patient care and services. This role requires a collaborative approach to ensure quality care, monitoring, and evaluation of services according to patient needs.

Key Responsibilities
  • Coordinate patient care and services, working closely with patients, families, care teams, payers, and external entities.
  • Develop and implement individualized care plans, promoting safe transitions from facility to next provider or care setting.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and services.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Ensure compliance with all applicable laws, regulations, and standards.
Requirements
  • Nursing diploma (associate's or bachelor's degree in nursing)
  • Currently licensed/registered in applicable State. Must maintain an active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing throughout employment.
  • One (1) year of clinical experience in post-acute care setting preferred
  • Prior case management, utilization review, and discharge planning experience preferred
  • Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
