Job Summary
The RN or LPN Case Manager is a critical role in our organization, responsible for coordinating patient care and services to ensure seamless transitions and optimal outcomes. As a key member of our care team, you will work closely with patients, families, physicians, and external entities to promote quality care and safety.
Key Responsibilities
- Coordinate patient care and services to ensure timely and effective transitions
- Develop and implement individualized care plans to meet patient needs
- Collaborate with interdisciplinary teams to promote quality care and safety
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
- Monitor and evaluate patient progress to ensure optimal outcomes
- Identify and address potential barriers to care to ensure patient safety and satisfaction
- Participate in quality improvement initiatives to enhance patient care and services
Requirements
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State. Must maintain an active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing throughout employment.
- One (1) year of clinical experience in post acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
- Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
