Job Title: Physician - Hospice Care Specialist

Care Synergy Medical Group is seeking a compassionate and skilled Physician to join our team as a Hospice Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality medical care to patients with serious illnesses, focusing on symptom management, pain control, and improving quality of life.

Responsibilities:
  • Provide physician services consistent with hospice and palliative care philosophy, consulting with attending physicians and hospice staff as needed.
  • Review clinical information upon patient referral to document disease diagnosis, prognosis, therapeutic regimen, and assessment of patient's health.
  • Participate in Interdisciplinary Group/Team (IDG or IDT) patient care meetings, utilizing review of patient care as necessary.
  • Determine clinical appropriateness of hospice and/or palliative admissions, transfers from palliative care to hospice, and discharges of hospice or palliative patients as needed.
  • Assist the President with program philosophy and service model for hospice & palliative care.
  • Follow hospice program policy when providing supervision for cross-coverage of physician service.
  • Serve as liaison with attending physicians, local hospitals, medical schools, and teaching hospitals, providing support to attending and consulting physicians, and overseeing patient medical services.
Requirements:
  • Minimum Education: Graduate from an approved school of medicine.
  • Minimum Experience: Three years' experience as a practicing physician in hospice, internal medicine, family practice, pediatrics, oncology, or a related field.
  • Required License: Current Colorado Physician License to practice medicine, Board certified (or board eligible) in Internal Medicine or Family Practice or Hospice & Palliative Medicine, Colorado state driver's license, and proof of insurance.
