Hospice Care Physician

Care Synergy Medical Group is seeking a dedicated Physician to provide exceptional medical services within the realm of hospice and palliative care.
Location:
Ft Collins, CO
Employment Type:
Part-time, 20 hours per week, Monday to Friday with some weekend availability.
Compensation:
Annual salary ranging from $106,912 to $125,621, with additional supplemental pay based on position and schedule, including paid mileage and shift differentials.

Key Responsibilities:
The Physician will:
- Deliver medical services aligned with hospice and palliative care philosophies.
- Guide and support the interdisciplinary team, ensuring quality and excellence in patient care.
- Consult with attending physicians and staff regarding patient care goals, pain management, and symptom relief.
- Participate in patient care meetings and reviews to ensure clinical appropriateness of admissions and discharges.
- Conduct home and facility visits as necessary, providing on-call support as needed.
Qualifications:
- Graduate from an accredited medical school.
- A minimum of three years' experience as a practicing physician in relevant fields.
- Current Colorado Physician License and board certification in Internal Medicine, Family Practice, or Hospice & Palliative Medicine.
- Valid CPR certification.
Physical Requirements:
Ability to lift a minimum of 35 lbs.
