Job Description

Job Summary:

The Resident Care Coordinator is a vital member of our team at The Vistas at Oxnard Senior Living, responsible for ensuring the daily health and well-being of our residents. This role requires a compassionate and organized individual who can effectively coordinate care services, supervise staff, and maintain accurate records.

Key Responsibilities:

  • Care Coordination: Review and document daily communication logs, new orders, and medication administration records to ensure seamless care delivery.
  • Staff Supervision: Supervise and train resident assistants, medication delivery staff, and other caregivers to ensure high-quality care.
  • Medication Management: Supervise the transcription process, medication ordering, and pharmacy cycle fill to ensure accurate and timely medication delivery.
  • Communication: Coordinate with outside agencies, medical providers, and family members to ensure resident needs are met.
  • Incident Reporting: Review and document incident reports to identify areas for improvement.
  • Cleanliness and Organization: Ensure the medication room and carts are clean and organized.
  • Resident Care: Review and assist with resident care plans, coordinate service plan reviews, and respond to Community RN audits.
  • Training and Development: Assist with new employee orientation, conduct exit-training interviews, and participate in on-call duties.
  • Other Duties: Perform other duties as assigned by management to ensure the smooth operation of our community.

Requirements:

  • Leadership Skills: Ability to utilize and apply basic management skills, including time management, prioritization, and delegation.
  • Care Coordination: Provides and coordinates resident care utilizing the principles of assisted living per state regulations.
  • Attendance and Communication: Maintains adequate attendance records, notifies supervisors of late or absent status, and communicates effectively with residents, staff, and family members.
  • Flexibility and Adaptability: Requires minimal supervision, is a self-starter, and flexible in a fast-paced environment.
  • Confidentiality and Respect: Keeps resident information confidential, respects resident rights, and maintains a professional demeanor.
  • Teamwork and Collaboration: Works cooperatively with supervisors and co-workers to achieve community goals.
  • Physical Demands: Ability to spend long periods on feet, ambulate quickly, and lift 40-50 pounds regularly.

Preferred Qualifications:

  • Experience with the Elderly Population: Previous work with seniors is desirable.
  • Medication Assistant Experience: Previous experience as a medication assistant is desirable.
  • CPR Certification: Maintains current CPR certification.

Universal Precautions: Exposure to blood/bodily fluids is unlikely.
