Job Summary

We are seeking a skilled Physical Therapist Assistant (PTA) to join our team of healthcare professionals in providing high-quality, personalized care to patients in their homes. As a Home Health PTA, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that address the unique needs and goals of each patient.

Key Responsibilities:

  • Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
  • Work collaboratively with the home health team to develop and implement individualized plans of care
  • Conduct assessments, document progress, and modify care plans as needed
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their plan of care, established goals, exercises, and self-care
  • Coach family members on the plan of care, therapy goals, and safely assisting with exercises

Requirements:

  • Active PTA license in Maine
  • Minimum of 1 year of PTA experience, ideally in home healthcare
  • CPR certification
  • Up-to-date on all State of Maine required vaccinations
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion
