About the Role

As a Home Health Physical Therapist Assistant, you will play a vital role in delivering personalized therapy to patients in their homes. Our team at Interim HealthCare of New Hampshire is dedicated to providing exceptional care and support to our patients, and we are seeking a skilled and compassionate PTA to join our team.

Key Responsibilities:

  • Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
  • Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans
  • Conduct assessments, document progress, and modify care plans as needed to ensure optimal patient outcomes
  • Educate patients and their families on therapy goals, exercises, and self-care techniques to promote independence and empowerment

Requirements:

  • Active PTA license in the State of Maine
  • Minimum of 1 year of PTA experience, preferably in home healthcare
  • CPR certification
  • Up-to-date on all State of Maine required vaccinations
  • Knowledge of state and federal home health regulations
  • Strong clinical judgment, excellent interpersonal skills, and a compassionate approach to patient care
