Job Title: Home Health Physical Therapist Assistant
Job Summary: We are seeking a skilled Physical Therapist Assistant to join our team of healthcare professionals at Interim HealthCare of New Hampshire. As a Home Health PTA, you will provide personalized home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury.
Key Responsibilities:
- Provide home-based physical therapy to patients with a focus on personalized care and outcomes.
- Work as part of a home health team to develop and implement patient care plans.
- Conduct assessments, document progress, and modify care plans as needed.
- Educate patients and their families on therapy goals, exercises, and self-care.
Requirements:
- Active PTA license in the State of New Hampshire.
- Minimum of 1 year of PTA experience, ideally in home healthcare.
- CPR certification.
- Up-to-date on all State of New Hampshire Required vaccinations.
- Knowledge of state and federal home health regulations.
