Job DescriptionAs a Home Health Physical Therapist Assistant with Interim HealthCare of New Hampshire, you will have the opportunity to make a meaningful impact in the lives of patients who require personalized therapy in the comfort of their own homes. Our team of healthcare professionals is dedicated to providing high-quality care that is tailored to each patient's unique needs and goals.

With a focus on patient-centered care, you will work closely with patients to develop and implement customized therapy plans that address their specific strengths, weaknesses, and mobility challenges. As a member of our interdisciplinary team, you will collaborate with nurses, occupational therapists, and other healthcare professionals to ensure that patients receive comprehensive and coordinated care.

As a Home Health Physical Therapist Assistant, you will have the flexibility to work in a variety of settings, including patients' homes, rehabilitation centers, and skilled nursing facilities. Key Responsibilities:

  • Develop and implement customized therapy plans for patients in their homes
  • Collaborate with interdisciplinary team members to ensure comprehensive and coordinated care
  • Provide hands-on therapy and education to patients and their families
  • Conduct assessments and document progress to inform treatment plans
  • Work with patients to establish and achieve their therapy goals

Requirements:

  • Active PTA license in the State of New Hampshire
  • Minimum of 1 year of PTA experience, ideally in home healthcare
  • CPR certification
  • Up-to-date on all State of New Hampshire required vaccinations
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion
