Case Manager, Harlem

Job Summary:
Graham Windham is seeking a skilled Case Planner to join our Foster Care program in Harlem. As a Case Planner, you will work closely with families and children to create and implement service plans that address safety, behavioral, educational, psychological, and permanency needs.
Responsibilities:
- Develop and implement service plans in partnership with families and children.
- Monitor safety, well-being, and progress against critical outcomes through regular casework contacts and documentation.
- Collaborate with internal and external service providers and stakeholders through clear and timely communication.
- Participate in Family Court Hearings, Family Team Conferences, and other meetings to advance permanency for children and families.
- Perform other duties as assigned.
Qualifications:
Requires a BSW or BA degree in Social Work, Sociology, Psychology, or a related field. Bilingual in Spanish is preferred. Must be able to travel within the NYC Metropolitan area and have excellent verbal and written communication skills, time management, and organizational skills. Computer literate, MS Office, and NYS Connections are required. Ability to work with diverse populations is essential.
