Foster Care Specialist, Harlem

Job Summary:
Graham Windham is seeking a skilled Case Planner to join our Foster Care team in Harlem. As a Case Planner, you will play a critical role in maintaining strong social contracts that emphasize family permanency planning and ensuring that every child is living in a nurturing, safe, and permanent family as quickly as possible.
About the Position:
The Case Planner will work closely with families, service providers, and other stakeholders to create and implement service plans that address safety, behavioral, educational, psychological, and permanency needs. This includes monitoring safety, well-being, and progress against critical outcomes, collaborating with internal and external service providers, and participating in Family Court Hearings and other meetings.
Responsibilities:
- Create service plans in partnership with families and children to address safety, behavioral, educational, psychological, and permanency needs.
- Monitor safety, well-being, and progress against critical outcomes through regular casework contacts and documentation.
- Collaborate with internal and external service providers and stakeholders through clear and timely verbal and written communication.
- Participate in Family Court Hearings, Family Team Conferences, and other meetings to advance permanency for children and families.
- Perform other duties as assigned.
Qualifications:
We are seeking a candidate with a BSW or BA degree in Social Work, Sociology, Psychology, or a related field. Bilingual Spanish is preferred. The ideal candidate will have excellent verbal and written communication skills, time management, and organizational skills. Computer literacy and experience with MS Office and NYS Connections are also required.
