Berkshire Nursing and Rehabilitation Center LPN (Med Pass) Job

We are seeking a reliable and compassionate LPN to join our team at Berkshire Nursing and Rehabilitation Center in West Babylon, NY.

About the Job:

This is a full-time LPN (Med Pass) position working in a skilled nursing and rehabilitation facility. The ideal candidate will have experience in performing a variety of care practices under the direction of the Registered Nurse and/or Unit Supervisor.

Responsibilities:

  • Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
  • Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
  • Document patient information, medical history, and treatment plans accurately and thoroughly.
  • Assist with case management and coordination of care for patients with complex medical needs.
  • Conduct physical examinations and assessments to determine patient health status.
  • Provide education and support to patients and their families regarding healthcare management.
  • Administer basic life support and emergency care as needed.

Requirements:

  • Must have a valid LPN license in the state of NY.
  • Experience working in a hospital or healthcare setting preferred.
  • Proficiency in using electronic medical record systems, such as Athenahealth.
  • Knowledge of medical coding and documentation practices.
  • Familiarity with occupational health procedures and protocols.
  • Ability to suture wounds and perform basic wound care.

Work Environment:

  • Long term care
  • Rehabilitation center

Shift and Schedule:

  • Standard shift: 3:30pm to 7:30pm
  • Rotating weekends

Relocation:

  • West Babylon, NY 11704: Relocation required before starting work
