Licensed Practical Nurse

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Charge Nurse. As a Charge Nurse, you will be responsible for providing high-quality patient care, leading a team of nursing staff, and ensuring the smooth operation of our nursing unit.
Key Responsibilities:- Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
- Lead and supervise a team of nursing staff, including LPNs, CNAs, and other support staff.
- Develop and implement individualized care plans for patients, in collaboration with the interdisciplinary team.
- Conduct regular rounds to assess patient needs and ensure the delivery of high-quality care.
- Collaborate with other healthcare professionals to ensure seamless communication and coordination of patient care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Maintain accurate and up-to-date patient records, including medication administration records and treatment plans.
- Ensure compliance with regulatory requirements, including HIPAA and OSHA standards.
- Current licensure as a Licensed Practical Nurse (LPN) in the State of New York.
- Minimum of 1 year of experience as an LPN in a long-term care or rehabilitation setting.
- Strong leadership and communication skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to prioritize tasks, manage multiple responsibilities, and maintain a high level of productivity in a fast-paced environment.
- Basic computer skills, including proficiency in electronic medical records (EMRs) and other healthcare software.
