Job Title: Licensed Practical Nurse

Job Summary:

The Licensed Practical Nurse (LPN) is a vital member of our healthcare team at Berkshire Nursing and Rehabilitation Center. As a LPN, you will be responsible for providing high-quality patient care under the supervision of a Registered Nurse and/or Supervisor. Your duties will include administering medications, taking charge of a unit, and performing various care practices.

Responsibilities:

  • Administer medications as ordered and maintain accurate records.
  • Take charge of a unit and ensure smooth operation of resident care.
  • Perform various care practices, including taking vital signs and documenting patient information.
  • Collaborate with the healthcare team to ensure effective patient care.
  • Assist with feeding residents during meal times and maintain a clean and safe environment.
  • Document patient information and maintain accurate records.
  • Participate in in-service classes and staff meetings to stay up-to-date on best practices.
  • Assist with orienting new nursing personnel and provide guidance as needed.
  • Perform other nursing functions as designated by your supervisor.

Qualifications:

  • Current licensure by the State of New York.
  • Mentally and physically capable of performing job duties.
  • Possesses leadership and supervisory qualities.
  • Possesses observation skills that enable the recognition of condition changes in the resident.
  • Ability to coordinate all functions in the operation of a resident unit through effective team nursing assignments.
  • Maintain meaningful and accurate documentation of services provided.

Personal Appearance:

  • Required uniform is to be worn.
  • Wear clean, well-fitting uniform with name tag and title.
  • Shoes must be clean, white rubber soled, and nursing regulation.
  • Makeup must be in good taste and moderate in amount.
  • Hair, hands, fingernails must be neat, well-kept and clean.
  • Jewelry should be restricted to simple ring and wristwatch.
  • Proper, acceptable, personal hygiene must be maintained at all times.

The Licensed Practical Nurse will work in a fast-paced healthcare environment, interacting with patients, families, and healthcare team members. The LPN must be able to work effectively in a team setting and maintain a calm and professional demeanor in high-pressure situations.
