Nurse Practitioner or Physician Assistant

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team on a PRN/Part Time basis. As a Home Risk Assessment provider, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and providing personalized care.
Responsibilities- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
- Collaborate with Primary Care Physician (PCP) on patient education and provide follow-up.
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
- Masters Degree required or commensurate experience and satisfactory completion of NP licensure.
- Masters Degree required or commensurate experience and satisfactory completion of PA licensure.
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
- Have a current BLS, ACLS, or CPR certification.
- Have a minimum of 1 year experience as a Nurse Practitioner/Physician Assistant.
- Have strong computer skills and familiarity with EMR software.
- Possess excellent verbal and written communication skills with patients, clients, and colleagues.
- Have a positive and outgoing personality and be a team player.
- Have the ability to travel, a valid state driver's license, and be able to drive a car, proof of adequate automobile insurance coverage for the state of residence.
- Travel required to meet patients where they live.
