About the Role:

We are seeking a skilled Care Specialist to join our team at AuthoraCare Collective. In this full-time weekend position, you will provide vital support to patients and their families on our Inpatient Unit at our Burlington Hospice Facility.

Responsibilities:

  • Conduct resource assessments and develop personalized plans for patients
  • Provide intervention and assistance to the Social Work Department
  • Evaluate patient needs and create effective care plans
  • Manage case files and maintain accurate documentation
  • Collaborate with interdisciplinary teams to ensure seamless patient care

Requirements:

  • Bachelor's or Master's degree in social work required
  • Two years of experience in social work, preferably in mental health or medical settings
  • Valid driver's license and reliable transportation
  • Ability to travel to patients' homes within the service area
