Position Overview:

The LTSS Service Coordinator plays a crucial role in managing service coordination for a designated group of individuals within specialized programs. This position is primarily field-based, allowing for remote work while also requiring in-person meetings with clients and healthcare providers.

Key Responsibilities:
  • Conduct face-to-face assessments to identify individual needs, utilizing motivational interviewing techniques for effective evaluations.
  • Coordinate care for members with complex health needs, serving as the primary contact for interdisciplinary teams.
  • Manage the non-clinical aspects of care for individuals with chronic conditions to promote efficient use of long-term services.
  • Collaborate with members to document their service and support goals, engaging their chosen care teams, including family and service providers.
  • Identify opportunities for alternative service levels or additional waiver programs for members.
  • Provide mentorship and training for new staff, contributing to process improvement initiatives.
  • Submit necessary documentation for utilization management, ensuring alignment with individual care plans.
  • Report critical incidents to relevant agencies and assist in the appeals process for member grievances.
Qualifications:
  • A Bachelor’s degree and a minimum of two years of experience in a social work environment, or a combination of education and experience that provides equivalent knowledge.
  • Specific qualifications may vary based on state regulations and contractual obligations.
Preferred Skills:
  • Degree in a healthcare-related field is preferred.
  • Bilingual proficiency in English and Spanish is strongly preferred.
