Job Description

Arizona Home Care and Patient Care Infusion are seeking a skilled Transitional Care Coordinator to represent us in central Phoenix hospitals and facilities.

The successful candidate will be responsible for the accurate and timely processing of referrals for Arizona Home Care (homecare) and Patient Care Infusion (home infusion).

Key responsibilities include maintaining lasting relationships with decision makers, referral sources, and physicians, as well as connecting with customers to understand their needs and demands.

The ideal candidate will possess strong knowledge and expertise on all service lines and be able to perform other duties as assigned.

Meet or exceed revenue goals for the Central Territory.

Requirements:

  • Minimum 1 year of work experience in a discharge planning role from a facility or hospital or at least 1 year of experience in a Medicare-certified, accredited home health agency, Infusion Pharmacy or related area
  • Current driver's license
  • Current vehicle insurance
