- Develop and implement individualized care plans for patients, taking into account their physical, psychosocial, and environmental needs.
- Communicate effectively with patients, families, and healthcare teams to ensure timely and accurate information exchange.
- Manage referrals and coordinate home care services, ensuring compliance with regulatory guidelines and agency policies.
- Provide education and support to patients and families, promoting self-care and independence.
- Participate in quality improvement initiatives and contribute to the development of best practices in home care.
- Current, valid RN license from the New York State Department of Education.
- Associate's or Bachelor's degree in nursing.
- Minimum 2 years of experience in community health, home care, or hospital settings.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
The Home Care Coordinator will work primarily in an office setting, with occasional visits to patients' homes. The working conditions are classified as light work, requiring the ability to sit for extended periods and perform tasks that involve minimal physical exertion.
