Job Summary

Key Responsibilities
  • Collaborate with hospitals and healthcare providers to prevent patient rehospitalization and ensure safe home care services.
  • Develop and implement individualized care plans for patients, taking into account their physical, psychosocial, and environmental needs.

  • Manage referrals and coordinate home care services, ensuring compliance with regulatory guidelines and agency policies.
  • Provide education and support to patients and families, promoting self-care and independence.

Requirements
  • Current, valid RN license from the New York State Department of Education.
  • Associate's or Bachelor's degree in nursing.
  • Minimum 2 years of experience in community health, home care, or hospital settings.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
