About the Role

We are seeking an experienced Registered Nurse with strong clinical skills and excellent communication abilities to join our medical team. As a Nurse Care Coordinator - Medical Specialist, you will play a vital role in ensuring seamless transitions of care and providing expert support to patients, families, and healthcare teams.

Key Responsibilities:
  • Conduct comprehensive assessments and develop personalized care plans to address complex medical needs.
  • Collaborate with multidisciplinary teams to ensure timely and effective care coordination, including referrals, transfers, and follow-up appointments.
  • Provide education and guidance to patients and families on self-care techniques, medication management, and disease prevention strategies.
  • Maintain accurate records and reports, utilizing electronic health records and other tools to enhance care delivery and outcomes.
Requirements

To be successful in this role, you must have:

  • A current license to practice as a Registered Nurse in the State of Tennessee.
  • A minimum of 2 years of experience in a hospital or healthcare setting, preferably in a critical care or telemetry unit.
  • Advanced clinical skills and knowledge of nursing best practices, particularly in areas such as trauma, pediatrics, or oncology.
  • Excellent communication, interpersonal, and problem-solving skills to effectively interact with diverse populations, including patients, families, physicians, and healthcare staff.
  • The ability to work independently and collaboratively as part of a high-performing team, prioritizing tasks, managing multiple responsibilities, and maintaining flexibility in response to changing demands.
Compensation and Benefits

We offer a competitive salary range of $85,000 - $120,000 annually, depending on experience, plus a comprehensive benefits package that includes:
