Job Description

Care Coordination Director

Elliot Health System is seeking a highly skilled and experienced Care Coordination Director to lead our Care Coordination team. As a key member of our leadership team, you will be responsible for developing and implementing strategic plans to improve patient outcomes, reduce costs, and enhance the overall quality of care.

Key Responsibilities:

  • Develop and implement a comprehensive Care Coordination strategy that aligns with the organization's goals and objectives
  • Lead a team of Care Coordinators to ensure seamless patient transitions and effective care coordination
  • Collaborate with clinical leaders to improve performance in payor designated quality outcomes and contractual obligations related to care coordination
  • Develop and maintain relationships with community providers and services to ensure optimal patient care
  • Monitor and analyze data to identify trends and areas for improvement
  • Develop and implement process improvements to optimize care coordination

Requirements:

  • Master's degree in Nursing, Business Administration, Health Administration, Social Work, or related field
  • Five years of progressive experience in Care Coordination leadership
  • Valid Nursing or LICSW credentials
  • Nationally recognized care management certification preferred
