MDS Coordinator Job Description

Manchester Center for Rehabilitation and Healing is seeking a skilled and motivated MDS Coordinator to join our dynamic team. The ideal candidate will possess a passion for geriatric care, a license in the state of practice, and the ability to evaluate and treat residents independently without direct supervision.

Key Responsibilities:
  • Oversee the accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments (CAAs), and Care Plans in accordance with federal and state regulations.
  • Act as an in-house Case Manager, demonstrating detailed knowledge of residents' health status and critical thinking skills to develop an appropriate care pathway.
  • Proactively communicate with the Administrator and Director of Nursing to identify regulatory risks, clinical trends, and operational outcomes.
  • Capture the RUG score, which reflects the care and services provided, and ensure timely electronic submission of all Minimum Data Sets to the state database.
  • Facilitate the Care Management Process, engaging residents, IDT, and family in timely identification and resolution of barriers to discharge.
  • Directly educate or provide company resources to IDT members to ensure they are knowledgeable of the RAI process.
  • Analyze QI/QM data in conjunction with the Director of Nursing Services to identify trends on a monthly basis.
  • Responsible for timely and accurate completion of Utilization Review and Triple Check.
Qualifications:
  • Licensed Nurse with current, active license in the state of practice.
  • Minimum two (2) years of clinical experience in a healthcare setting.
  • Minimum of one (1) year of experience in a long-term care setting.
  • Prior experience as an MDS Coordinator is accepted.
