Position Overview:

RMD VillageHealth DM, LLC is seeking a dedicated Full-Time Registered Nurse Case Manager (RN) to enhance our team. This role is pivotal in delivering comprehensive care to patients with complex health needs.

Compensation:

The position offers a competitive salary range of $90,000-$110,000 annually, along with a $10,000 retention bonus.

Key Responsibilities:

  • Provide exceptional support to patients navigating the healthcare system.
  • Conduct thorough medical record reviews to assess the medical, social, emotional, and financial needs of patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
  • Work independently and collaboratively with the healthcare team to ensure high-quality, cost-effective care.
  • Establish and maintain empathetic relationships with patients and their families.
  • Coordinate comprehensive care plans, including managing transitions of care and ongoing patient needs.
  • Identify and address social determinants of health impacting patient care.
  • Monitor patients for unreported medical conditions that may lead to adverse outcomes.

Qualifications:

  • Current RN License is required; BSN is preferred.
  • A minimum of 5 years of nursing experience is essential.
  • Current CPR or BLS certification is mandatory.
  • Possess a valid driver's license and be willing to travel within the designated area.
  • Intermediate proficiency in computer applications, including MS Word, Excel, and Outlook.
  • A home office setup with reliable internet connectivity is required.

Preferred Skills:

  • Experience in Case Management or Chronic Care Management for 2+ years.
  • Utilization management experience of 1+ years is advantageous.
  • Spanish-speaking candidates are preferred.
