RN Case Manager

MMP Staffing Services is seeking a dedicated and compassionate Registered Nurse (RN) Case Manager to coordinate care for patients, collaborate with healthcare providers, and ensure continuity of care throughout the healthcare continuum.
Key Responsibilities:
- Care Coordination: Assess patients' healthcare needs and develop individualized care plans in collaboration with interdisciplinary teams.
- Patient Advocacy: Advocate for patients' rights, preferences, and needs within the healthcare system.
- Utilization Review: Conduct utilization reviews to optimize healthcare resources and ensure cost-effective care.
- Documentation and Reporting: Maintain accurate and thorough documentation of patient assessments, care plans, interventions, and outcomes.
Qualifications:
- Education: Bachelor of Science in Nursing (BSN) from an accredited institution; Master's degree preferred.
- Licensure and Certification: Current licensure as a Registered Nurse (RN) in Maryland, Virginia, and/or Washington DC; Certification in Case Management (CCM) or related specialty preferred.
- Experience: Minimum of 2 years of clinical nursing experience, preferably in case management or care coordination.
Work Environment:
- The position requires working in a clinical and office setting, with occasional travel to patient homes or healthcare facilities.
