Behavioral Health Therapist

At People's Community Clinic, we are seeking a highly skilled and compassionate Behavioral Health Therapist to join our team. As a key member of our Integrated Behavioral Health Program, you will play a vital role in providing therapeutic services and social support to our patients.
Responsibilities:- Lead and manage the Integrated Behavioral Health Program, including clinical services and supervision, quality assurance, and departmental metrics.
- Work with the Chief Medical Officer and Chief Operating Officer to set productivity goals and develop realistic plans to achieve them.
- Provide clinical supervision for LMSW staff seeking LCSW licensure and ongoing supervision of LCSWs and IBH Program Managers.
- Participate in cross-departmental comprehensive case staffings and serve as a Social Service/Behavioral Health Representative on the Peer Review Committee.
- Assess capacity within and across departments and coordinate relevant training opportunities to increase staff skill and comfort in working with mental health.
- Coordinate changes in workflows and procedures within the IBH Team and as they relate to other departments.
- Assist in data collection and manage reports for internal review and to identify and address potential growth areas.
- Work with the Departmental Operations Coordinator to help support team productivity goals and give input for template development.
- Collect data and prepare reports as required by funding sources and PCC management.
- Provide direct services, including therapeutic intervention and social services to patients experiencing mental health issues.
- Conduct mental health screenings and onsite consultations, complete intake assessments, and provide brief crisis assessments and interventions.
- Engage in collaboration among cross-functional teams to provide coordinated trauma-informed and person-centered care to patients with complex biopsychosocial needs.
- Document services provided to clients per guidelines and policies.
- Provide follow-up to patients to assess suitability of and follow through with referrals.
- Identify opportunities to improve workflow, work processes, and patient care, and work cooperatively to implement these and other quality improvements.
- Master's Degree in Social Work.
- Five years of experience as a clinical social worker, preferably in a hospital, community health center, or other multidisciplinary setting.
- Five years of supervisory experience.
- Grant-writing and management experience required.
- Clinical experience working directly with individuals and their families.
- LCSW-S required.
- 18 PTO days per year & 11 paid holidays.
