OVERVIEW:
Under the direct supervision of the Crisis Outreach Program Manager, the Lead Wellness Coordinator will be primarily responsible for executing and sustaining ongoing operations related to assigned health and wellness initiatives.
The Lead Wellness Coordinator is expected to adhere to best practice guidelines to foster a secure and stable environment for all individuals served.This role demands a high level of ethics, compassion, professionalism, commitment, flexibility, and adaptability to effectively uphold the mission and values of the Coalition for the Homeless of Central Florida while maintaining an atmosphere of dignity, respect, and collaboration.
EDUCATION AND EXPERIENCE
A Bachelor's degree in healthcare, Nursing, Public Health, or related fields is required, along with 3 to 5 years of relevant experience in the healthcare sector, specifically in the coordination of health services.
Bilingual candidates are highly encouraged to apply.Prior experience in a community health center or primary care environment is preferred.
Three to five years of experience in coordinating and managing health and/or educational programs is advantageous.
PRIMARY RESPONSIBILITIES
Collaborate with staff to ensure a holistic and comprehensive healthcare experience for all individuals served.
Implement and sustain general health promotion initiatives that are innovative, supportive, and effective for the program participants.
Assist individuals with mental health challenges, medical impairments, and/or co-occurring substance use disorders in applying for Social Security Administration's (SSA) disability programs: Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI).
Follow up with individuals to provide ongoing support and ensure access to essential resources.
Coordinate Health and Wellness activities, including education, assigning tasks, reviewing, and evaluating processes and performance.
Assist with scheduling wellness screenings, including some external coordination for services.
Oversee all agency entry points for health/prevention measures and protocols.
Participate in the development of appropriate standards for work processes.
Monitor guest wellness tracking and health screenings.
Prepare recommendations for assigned projects/program changes and/or prioritization as needed or requested.
Facilitate educational groups related to best practices and prevention.
Ensure that adequate information regarding medical/health insurance opportunities is accessible to individuals served, including brochures, flyers, signs, posters, etc.
Adhere to all program criteria and utilize an electronic reporting system to track progress and planning.
Perform other duties as required by the supervisor based on program evolution or immediate programmatic needs.
Exemplify the Coalition's mission statement and adhere to agency policies and procedures.
ESSENTIAL REQUIREMENTS
Knowledge of local healthcare service providers, educational, and prevention programs available in the community.
Excellent verbal, written, and presentation skills.
Ability to work independently with minimal supervision.
Strong organizational skills and accountability.
Ability to establish and maintain effective relationships with diverse groups, including individuals at all levels both within and outside the organization, and gain their cooperation.
Willingness to work flexible hours, including evenings and weekends.
