Wellness Care Specialist

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at Somerby Lake Nona. As a Licensed Practical Nurse, you will play a vital role in providing high-quality care to our residents, ensuring their physical, emotional, and social needs are met.
Key Responsibilities:- Implement and coordinate the delivery of care in collaboration with physicians and care associates.
- Assess, plan, implement, and evaluate resident care plans.
- Provide high-level care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing residents' overall condition and behavior.
- Maintain ongoing communication with residents, families, and physicians regarding resident care.
- Assist the Director of Wellness in the development and update of resident care plans.
- High school diploma or equivalent (GED)
- LPN License in applicable state
- First Aid & CPR Certified
- At least 1 year of related work experience
