Pediatric Transport Nurse Coordinator

We are seeking a highly skilled Registered Nurse Coordinator to join our Pediatric Transport Team at The Hospitals of Providence - Memorial Campus. As a key member of our team, you will be responsible for coordinating the care needs of assigned patients and developing comprehensive plans to manage care delivery across the patient care continuum.
Responsibilities- Collaborate with physicians, nurses, allied health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion.
- Advocate for patients and balance care needs and financial considerations to ensure efficient and effective treatments are achieved.
- Develop customized care plans based on individual patient needs and preferences.
- Partner with patients and their primary physicians to ensure seamless care delivery.
- Academic degree in nursing.
- 2 years of nursing experience.
- Current licensure, certification, or registration to practice as a registered nurse.
