Job Description

Position Summary:

The Direct Service Provider will provide specific services to members based on their Service Plan. As a key member of our team, you will help individuals reach goals set forth in their Service Plan by offering assistance and interventions that promote independence and growth.

Main Responsibilities:

  1. Cultivate an understanding and appreciation for diversity, ensuring services are sensitive to the cultural, social, and individual contexts of each child/youth and their caregiver/family.
  2. Provide educational resources, advocacy, and support tailored to the unique circumstances of each family.
  3. Enhance the child's integration into their family unit and bolster the caregiver's capacity to support them in home and community settings.
  4. Empower families to access resources independently, promoting long-term self-sufficiency.
  5. Address any concerns related to supporting their child, respecting their cultural and individual contexts.
  6. Offer comprehensive instruction to deepen understanding of the child's condition or illness.
  7. Engage with families and children to promote a holistic approach to accessing community services and activities.
  8. Regularly update service plans, ensuring they reflect evolving member needs.
  9. Attend waiver service provider meetings to align with best practices and protocols.
  10. Report incidents promptly to supervisors.
  11. Collaborate with other service providers to track progress, address barriers, and streamline referrals.
  12. Adhere to documentation requirements, submitting progress notes, service plans, expense logs, and service provider logs in a timely manner.
