We are currently seeking a compassionate and dedicated Per Diem Nurse Practitioner to join our team. The coverage area for this position is Wilmington and surrounding areas.

Prior home care, hospice, or palliative care experience is strongly preferred but not required. As a hospice nurse practitioner, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Nurse Practitioner Responsibilities:

  • Making home face-to-face hospice recertification visits to clients in designated geographic territories.
  • Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
  • Following up with, executing, and properly documenting doctors' orders.
  • Performing client assessments as necessary.

Salary Information:

The estimated salary range for this position is $80-$110 per hour based on industry standards and location. Salary may vary depending on qualifications and experience.

Qualifications:

  • Current licensure as a Nurse Practitioner in Delaware.
  • A minimum of one year of recent, verifiable clinical experience.
  • Prior home care, hospice, and/or palliative care experience is strongly preferred but not required.
  • Solid computer skills; prior experience with electronic medical records preferred.
  • Ability to travel to cases as assigned.
  • Strong interpersonal skills.
