Job Overview

Connexzia seeks an experienced Clinical Staff Pharmacist to fill the role of Night Lead, leading a two-pharmacist team during overnight hours in our inpatient pharmacy.

About the Role

This position is responsible for providing expert pharmaceutical care, ensuring medication safety, and optimizing patient drug therapies. As a Night Lead, you will be part of a multidisciplinary care team and provide leadership support to staff during overnight shifts.

Key Responsibilities

  • Interpret and validate medication orders to ensure safety and appropriateness.
  • Perform order verification, IV to PO conversion, and other core clinical activities to promote effective patient care.
  • Monitor patient drug therapies and provide consultation to medical staff to ensure optimal treatment outcomes.
  • Assist with medication histories and ensure safe, effective drug administration to patients.
  • Provide leadership and direction to support staff during overnight shifts, promoting a culture of excellence and teamwork.
  • In the absence of a supervisor, assume essential leadership responsibilities to ensure continuity of care.

Shift Information

  • Work onsite as part of a two-pharmacist team during overnight hours, collaborating with colleagues to ensure seamless patient care.
  • Rotate weekends and holidays as part of the inpatient pharmacy schedule, demonstrating adaptability and a commitment to meeting the needs of the organization.

Patient Populations Supported

  • Oncology, surgery, cardiology, pediatrics, adult and pediatric critical care, peri-operative, psychology, same-day infusions, renal, and rehabilitation patients receive exceptional care from our team.

Education and Experience

  • A Doctor of Pharmacy (Pharm.D.) degree or Bachelor of Science in Pharmacy from an ACPE-accredited institution is preferred.
  • Two years of clinical pharmacist experience in a health system, preferably in a similar role, is highly valued.
  • ASHP-accredited Pharmacy Residency or equivalent experience is a plus.
  • Licensure: Must be a licensed pharmacist in Vermont, demonstrating a commitment to staying current with regulatory requirements and industry developments.

Salary and Benefits

We offer a competitive salary, estimated at $145,000 - $170,000 per year, depending on experience, as well as a partial relocation package to support your transition to our team. A signing bonus of $3,000 to $5,000 may also be offered, based on qualifications and experience.
