Job Title: Medical Social Worker - Home Health

Join CenterWell Home Health as a Medical Social Worker and play a vital role in providing comprehensive care to patients in their homes. As a member of our interdisciplinary care team, you will work closely with patients, families, and healthcare professionals to address the emotional, social, and environmental factors that impact patient outcomes.

Key Responsibilities:
  • Evaluate and develop personalized plans of care to meet the unique needs of each patient.
  • Assess and address the social and emotional needs of patients and their families, including access to community resources and support services.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Supervise and mentor Social Work Assistants to ensure high-quality patient care.
  • Document patient progress and outcomes in accordance with company policies and procedures.
Requirements:
  • Masters or doctoral degree in Social Work from a Council on Social Work Education-accredited program.
  • Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
  • Knowledge of community resources and ability to assist with discharge planning needs.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
