  • Assess and Address Patient Needs

Conduct thorough assessments of patients' social and emotional states, identifying areas where we can provide support and resources to aid in their recovery.

Develop Personalized Plans of Care

Collaborate with our care team to create tailored plans of care that address the unique needs of each patient, ensuring they receive the support and services they require.

Connect Patients with Community Resources

Identify and connect patients with local resources, services, and support groups to help them navigate their healthcare journey and achieve optimal outcomes.

Provide Education and Support

Offer education and support to patients and their families, empowering them to make informed decisions about their care and well-being.

Requirements and Qualifications

To be successful in this role, you'll need:

  • Master's or Doctoral Degree in Social Work
  • Current Social Work Licensure in the State of Practice
  • Valid Driver's License and Reliable Transportation
  • Proof of Current CPR Certification
  • Minimum of One Year of Experience as a Social Worker in a Healthcare Setting
  • Excellent Communication and Interpersonal Skills
  • Ability to Work Independently and as Part of a Team
