  • Assess and Address Patient Needs

Conduct thorough assessments of patients' social and emotional states, identifying areas where we can provide support and resources to aid in their recovery.

Develop Personalized Plans of Care

Collaborate with our care team to create tailored plans of care that address the unique needs of each patient, ensuring they receive the best possible support and services.

Connect Patients with Community Resources

Help patients and families access community resources, such as housing, financial assistance, and other support services, to aid in their recovery and well-being.

Provide Education and Support

Offer education and support to patients and families, empowering them to make informed decisions about their care and well-being.

Requirements and Qualifications

To be successful in this role, you'll need:

  • Masters or Doctoral Degree in Social Work
  • Current Social Work Licensure
  • Valid Driver's License and Reliable Transportation
  • Minimum of One Year of Experience in Healthcare
  • Excellent Communication and Interpersonal Skills
