About the Role

We are seeking a skilled Certified Wound Ostomy RN to join our team at Providence Home Services Oregon. As a key member of our home health team, you will provide high-quality patient care and support to individuals in their homes, focusing on wound and ostomy management.

Responsibilities
  • Provide direct and indirect quality care for patients of all ages, following the nursing process and in accordance with the Nurse Practice Act.
  • Accept professional development as an integral part of nursing practice.
  • Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments, develop and implement wound and ostomy care plans, and educate patients and families on self-care techniques.
  • Collaborate with other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.
Requirements
  • Bachelor's Degree in Nursing from an accredited school of nursing.
  • Completion of an accredited course in Wound, Ostomy and Continence.
  • Current Oregon Registered Nurse License.
  • Current Washington Registered Nurse License (if practicing in Washington state).
  • Certification as a Enterostomal Therapist by the Wound Ostomy and Continence Nursing Certification Board within 90 days of hire.
  • 2 years of nursing experience, with a focus on wound and ostomy care.
