Position Overview:

Key Responsibilities:
1. **Patient Care:**
- Develop personalized nutrition care strategies for patients with diverse medical conditions.
- Conduct thorough assessments using established nutrition screening tools to identify patient needs.
- Collaborate with healthcare professionals, patients, and families to ensure effective nutrition management.
- Maintain accurate documentation of patient interactions and care plans in the electronic medical record system.

2. **Nutrition Education:**
- Provide nutrition education to patients, families, and community groups to promote healthy eating habits.
- Assist in the creation and updating of educational materials and resources for patient use.
- Evaluate the effectiveness of educational initiatives and adjust as necessary to meet patient needs.

Qualifications:
- Bachelor’s degree in Nutrition or Dietetics, or a Master’s degree in a related field preferred.
- 2 to 5 years of experience in a clinical or hospital environment is essential.
- Registered Dietitian credential from the Commission on Dietetic Registration is required.
- Certification as a Diabetes Educator is preferred.
- Must possess a valid driver’s license and be able to meet state residency requirements for service provision.

Physical Requirements:
This position may involve sedentary work, with occasional lifting of up to 10 lbs.
