Job Summary

We are seeking a highly skilled and compassionate Clinical Trauma Coordinator to join our team at Holmes Regional Medical Center. As a key member of our Trauma Department, you will play a vital role in providing exceptional care and support to trauma patients and their families.

Key Responsibilities
  • Initiate risk and injury prevention assessments for trauma patients and develop education plans to prevent further injury.
  • Collaborate with trauma surgeons, nurses, and healthcare teams to develop desired discharge goals.
  • Communicate discharge plans and options to patients, families, physicians, and healthcare teams.
  • Coordinate and document complex discharge plans, including injury prevention assessments and education.
  • Provide crisis intervention services to patients and families impacted by major trauma, acute grief, and catastrophic injuries/illnesses.
  • Assess and treat patients with substance abuse behaviors and coordinate referrals to treatment.
  • Identify clinical and system variations in standard care for trauma patients and participate in Performance Improvement Patient Safety (PIPS) processes.
  • Intervene and recommend referrals to resolve system barriers to patient discharge.
Requirements
  • Licensure: Current State of Florida LCSW license, MSW actively pursuing LCSW, or Current State of Florida RN license.
  • Work Experience: At least five (5) years of acute care experience, with three (3) years in a Level I/II Trauma Center.
  • Knowledge/Skills/Abilities: Ability to communicate effectively and respectfully with patients, families, and healthcare teams.
Preferred Qualifications
  • Education: If RN, BSN preferred.
  • Work Experience: Case Management experience, related management experience, and five (5) years of experience in a Level I/II Trauma Center.
  • Knowledge/Skills/Abilities: Highly developed verbal and non-verbal communication skills, experience in planning and implementing education and training, and experience with assessment and treatment of PTSD and acute stress symptoms.
Physical and Mental Demands
  • Able to stand, walk, sit, and travel large distances repeatedly within the hospital.
  • Able to lift up to twenty (20) pounds unassisted up to five (5) times a day.
  • Ability to hear and understand patient, family, and medical staff correctly.
  • Must be able to communicate with patients, families, and staff.
