Job OpportunityCase Manager Position

We are seeking a dedicated and compassionate Case Manager to join our team at the Developmental Disabilities Resource Board of St. Charles County. As a Case Manager, you will play a vital role in providing resources and opportunities for individuals with developmental disabilities and their families.

Key Responsibilities:

  • Develop and implement person-centered plans to support individuals with developmental disabilities
  • Collaborate with multi-disciplinary teams to ensure comprehensive care and services
  • Provide education and support to individuals and their families to promote independence and self-advocacy
  • Connect individuals with community resources and services to enhance their quality of life

Requirements:

  • Bachelor's degree in a human services field from an accredited college or university
  • Preferred experience working with individuals who have intellectual/developmental disabilities
  • Preferred experience in a multi-disciplinary team setting
