We are seeking an experienced Registered Nurse to join our team as a Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients and their families.

Responsibilities
  • Conduct initial assessments of patients and families to determine their care needs
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Provide education and support to patients and their families to promote self-care and independence
  • Manage and coordinate the care of patients with complex needs, including those requiring palliative care
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Graduate of an accredited school of nursing with a current RN license
  • One to two years of recent acute care experience in an institutional setting
  • Current CPR certification and valid driver's license
  • Bachelor's degree in nursing or related field preferred
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, basic computer skills, and nursing skills per competency checklist
