Job Title: Foster Care Case Manager

At KVC Health Systems, we are seeking a compassionate and dedicated Foster Care Case Manager to join our team. As a key member of our child welfare services, you will play a vital role in providing case management services to children and their families placed in out-of-home care through the court system.

Job Summary

The ideal candidate will have a strong understanding of working with children, excellent interpersonal and critical thinking skills, and a commitment to the well-being and positive development of children. Responsibilities
  • Create and execute permanency plans for children under the custody of the Children's Division.
  • Provide case management services to children and their families, including assessment, planning, and coordination of services.
  • Develop and maintain relationships with community resources and service providers to ensure access to necessary services.
  • Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care.
  • Document all interactions and services provided in accordance with agency policies and procedures.
Requirements
  • Bachelor's degree in social work, psychology, or a related human services field.
  • Prior experience working with children is essential, with experience in foster care or child welfare being advantageous.
  • Strong interpersonal skills and the ability to communicate effectively with diverse individuals and groups.
  • Critical thinking skills to assess situations and make informed decisions in the best interest of the child.
  • Knowledge of relevant laws, regulations, and best practices in the field of child welfare.
