Job Summary

We are seeking an experienced RN Case Manager to join our SSM Health float pool team. This individual will float to our 6 St. Louis locations, providing exceptional patient care and ensuring a smooth transition to the next level of care.

Key Responsibilities
  • Assess patients' discharge planning needs and develop a plan to meet those needs in collaboration with the interdisciplinary team.
  • Identify and escalate gaps/barriers related to inpatient and observation length of stay.
  • Participate in the utilization management process, ED case management process, and admission process.
  • Collaborate with post-acute providers, such as skilled nursing facilities, rehab, and home health, to ensure seamless care transitions.
  • Apply evidence-based practice and scientific knowledge to deliver patient-centered care.
  • Work in a constant state of alertness and safe manner.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensing.
  • Three years' acute care experience.
Physical Requirements
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near and far.
  • Frequent use of hearing and speech to share information through oral communication.
Professional Licenses and Certifications
  • Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR).
  • Registered Nurse (RN) - Missouri Division of Professional Registration.
  • Registered Nurse (RN) - Oklahoma Board of Nursing (OBN).
  • Registered Nurse (RN) - Wisconsin Department of Safety and Professional Services.
