This role encompasses conducting home assessments, engaging with elderly individuals or those with disabilities, documenting essential information, identifying service needs, and formulating service plans. Responsibilities also include verifying data, elucidating program benefits and requirements, referring clients to suitable services, and ensuring that authorizations are accurately recorded in automated systems.
The specialist is responsible for authorizing and overseeing Community Care Services Eligibility (CCSE) to confirm that services align with the needs of individuals.Additional responsibilities may involve acting as a supervisor in the absence of the unit leader, reviewing cases, offering guidance and policy interpretation to colleagues, training new personnel, or preparing specialized reports. Proficiency in computer operations is required, and participation in regional special projects may be expected.
Key Responsibilities:
- Assess eligibility for services in accordance with established program policies and procedures.
- Design and execute service plans tailored to client needs.
- Provide continuous case management to evaluate individual conditions and the effectiveness of services in addressing various issues.
- Maintain comprehensive case records to ensure a documented history for each client.
- Compile reports and records to furnish management with information pertinent to program operations.
- Assist the supervisor with administrative tasks, which may include acting as a temporary supervisor, organizing and leading staff meetings, delegating tasks, providing counseling, and monitoring staff performance and caseloads.
- Utilize computer systems to document, update, and retrieve case information.
- Clarify policies for staff members.
- Mentor and support staff development.
- Contribute to the training of new employees.
- Support the unit supervisor in report preparation or in resolving complex case issues.
- Serve as a resource for challenging or intricate cases.
- May review cases for the unit.
- May engage in recruiting and supervising volunteers.
- Perform related duties as assigned.
- Travel up to 65% of the time for home visits, training sessions, and meetings.
- Understanding of gerontology, the aging process, and the unique challenges faced by individuals in Long-Term Care Services.
- Familiarity with interviewing techniques to gather personal information, make inquiries, and resolve discrepancies.
- Strong oral and written communication skills.
- Proficient knowledge of the English language, including vocabulary, grammar, and effective composition for correspondence and case reporting.
- Ability to actively listen to clients, explore their feelings, and present relevant options regarding fees, bills, and potential outcomes.
- Capability to complete tasks within specified timeframes.
- Skill in evaluating living conditions, encompassing both physical and emotional environments.
- Ability to prioritize tasks based on importance and deadlines, set objectives, and manage time effectively.
- Competence in monitoring unit operations.
- Aptitude for observing employee performance and documenting findings.
- Ability to build rapport with clients, colleagues, contract agency personnel, and staff from other governmental agencies.
- Skill in communicating with community members to seek assistance for clients, foster support, and coordinate resources and programs.
- Ability to read and analyze cases accurately.
