Job Title: Clinical IV Pharmacist

Eppy's IV is seeking a dedicated pharmacist to join our infusion team in Beckley, WV. As a Clinical IV Pharmacist, you will play a vital role in providing patient-centered care tailored to each patient's medical needs regarding IV therapy.

Responsibilities:
  • Develop and manage IV therapy patient treatment plans, including outpatient parenteral antibiotic therapy, TPN therapy, biologic therapy, and specialty medication therapy.
  • Maintain accurate documentation of treatment plans and clinical monitoring of IV therapy.
  • Ensure compounding compliance with USP 797 and USP 800 regulations.
  • Collaborate with internal departments, home health agencies, healthcare facilities, and physicians to ensure seamless patient care.
  • Participate in quality control and performance improvement initiatives to maintain high standards of patient care, medication, and equipment usage.
  • Participate in the on-call rotation to ensure 24/7 patient care.
Requirements:
  • PharmD degree
  • Active WV Pharmacist License
