Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at LifeStream, Inc. as a Residential Care LPN. In this role, you will provide high-quality patient care and support to individuals with disabilities in their homes and communities.

Key Responsibilities
  • Provide medical care and support to individuals with disabilities in residential settings
  • Assist with daily living skills, including emotional and social support
  • Collaborate with healthcare teams to ensure comprehensive care
  • Document patient progress and maintain accurate records
Requirements
  • Current LPN license
  • Valid driver's license and access to a vehicle
  • Ability to pass employment screening process
